The State Water Board has identified the following violations pursuant to H&SC, chapter 6.7, and California Code of Regulations (CCR), title 23, chapter 16: No. Violation Tank Start Date Stop Date Regulation Failure to Maintain Operating Permit Onsite—A current June 13, 23 CCR 1 operating permit was not Unleaded 2019 Ongoing 2712(i) available at the time of inspection. Failure to Maintain Plot Plan/Site Map—An approved plot plan/site map indicating the location of the product piping, June 13, 23 CCR 2 tanks, and auxiliary equipment Unleaded 2019 Ongoing 2711(a)(8) with respect to buildings or other landmarks was not available in CERS or onsite at the time of the inspection. Failure to Maintain tank information—In CERS, under H&SC June 13, 25286(a); 4 tank construction, fill tube shut- Unleaded Ongoing going 23 CCR off valve lists"yes"; however, it should list"no." 2711(a) Failure to Maintain Monitoring Plan— In CERS, the continuous electronic tank monitoring description must be completed. Also, automatic tank gauging lists"yes"; however, it should list "no." In addition, the pipe H&SC monitoring description must be 25286(a); 5 completed. The UDC monitoring Unleaded June 13, Ongoing 23 CCR must be completed and list the 2019 2 32(d 1 , 2632 observed optical sensor. Under 9 periodic system testing, spill (ax ) bucket testing should list"yes." Under record keeping, equipment maintenance and calibration records should list "yes." Lastly, the training description must be completed. Failure to Maintain Owner/Operator Statement of Understanding and June 13, 23 CCR 6 Compliance—A current Unleaded Ongoing Owner/Operator Statement of 2019 2715(a) Understanding was not available at the time of inspection. Failure to Maintain Designated Operator(DO) UST Identification Form—The DO June 13, 23 CCR 7 UST Identification Form Unleaded 2019 Ongoing 2715(a) observed at the time of inspection was signed by a former employee. Employee Training Certificate. Failure to Perform DO Inspection-The DO inspection recorded on November 13, November November 13, 23 CCR 9 2018,was not performed within Unleaded 10, 2018 2018 2716(a) 30 days of the previous DO inspection recorded on October 10, 2018. Failure to Perform DO Inspection-The DO inspection recorded on June 3, 2019,was April 30, 23 CCR 10 not performed within 30 days of Unleaded 2019 June 3, 2019 2716(a) the previous DO inspection recorded on April 29, 2019. Failure to Perform Overfill Prevention Equipment inspection-The overfill 11 prevention equipment inspection Unleaded October 13, January 22, 23 CCR was due on October 13, 2018; 2018 2019 2637.2(ax1) however; the inspection was not performed until January 22, 2019. Failure to Monitor Product Piping-The sensor located inside the turbine sump is unable to detect a leak at the 12 earliest opportunity because the Unleaded June 13, Ongoing 23 CCR sensor is not located at the 2019 2630(d) lowest point in the sump. Additionally,the sensor located in the UDC was not oriented in a vertical position. You are directed to correct the ongoing violations and submit compliance documentation to the State Water Board and Fresno County Environmental Health within sixty(60) days from the date of this letter. Have your DO make specific notations in the next monthly DO report indicating the ongoing violations have been corrected. The monthly DO report and any associated photos must be submitted as proof of compliance. Please ensure that the contact information for all personnel related to the UST system is current. In addition, as least one of the facility employees present during operating hours shall have current training.
